The Continuum of Care

Specialized Care at Each Level

As health needs change, people require different levels of care. An independent person who suffers illness or injury may undergo hospitalization, followed by recovery in a rehabilitation center.

People who can no longer perform activities of daily living (i.e, bathing, dressing, taking medication or other tasks) usually move to an Assisted Living residence with caregiver support. This can sometimes include memory care for mild to moderate cognitive impairment.

People who need full-time nursing care and help with all activities of daily living move to a Skilled Nursing facility, which may include secure memory care residential living.

Assisted Living and Skilled Nursing are supported by Rehabilitation teams to help residents build and maintain skills.

While you can access all these services from separate facilities, there’s a benefit to turning to one provider who understands your big picture:
