Acceptance: Absolute Travel Getaway Sweepstakes

By submitting an entry with the Employee Satisfaction Survey, the participant hereby acknowledges and accepts the terms and conditions outlined in the “Terms and Conditions: Absolute Travel Getaway Sweepstakes” provided by Transitions Healthcare, LLC. By participating in the Sweepstakes and submitting an entry, the participant confirms that they have read, understood, and agreed to abide by all the rules and provisions set forth in the Terms and Conditions.

The participant understands that their participation is subject to eligibility requirements, the specified entry period, and the random selection process for the grand prize winner. The participant agrees to provide accurate and complete information in the survey, including my name, phone number, and email address. By accepting these terms and conditions, the participant affirms that they are eligible to participate, and agree to be bound by these terms and conditions for the Absolute Travel Getaway Sweepstakes.

By participating in the Sweepstakes and accepting the grand prize, the winner acknowledges and agrees to the terms and conditions of this non-exchangeable provision. By accepting the grand prize, the winners affirms that they have read, understood, and agreed to the non-exchangeable provision as outlined in the “Prize” section of the Terms and Conditions.

Furthermore, the participants acknowledge that the decision of Transitions Healthcare, LLC regarding the selection of the grand prize winner is final and binding. The participant grants Transitions Healthcare, LLC the right to use their name, likeness, and survey responses for promotional purposes in the event that they are selected as the winner, as specified in the Terms and Conditions.

The participant hereby releases Transitions Healthcare, LLC, its affiliates, officers, directors, employees, and agents from any and all liability for any injuries, losses, or damages arising from their participation in the Sweepstakes or their acceptance, possession, or use of the prize.

The participant understands that their personal information will be collected and used for the purposes of administering the Sweepstakes and contacting the winner, in accordance with Transitions Healthcare, LLC’s Privacy Policy.

By participating in the Sweepstakes, participants agree to abide by these Terms and Conditions and acknowledge that the Sweepstakes is in no way sponsored, endorsed, administered by, or associated with any third party.

Transitions Healthcare, LLC reserves the right to modify or terminate the Sweepstakes at any time without prior notice.

This document constitutes the complete and exclusive agreement between the participant and Transitions Healthcare, LLC with respect to the Sweepstakes.

Terms and Conditions: Absolute Travel Getaway Sweepstakes

Eligibility: The Absolute Travel Getaway Sweepstakes (the “Sweepstakes”) is open to all employees of Transitions Healthcare, LLC, who complete the Employee Satisfaction Survey within the Entry Period, and who are 18 years of age or older at the time of entry. Officers and Directors of Transitions Healthcare, LLC, and their immediate family members are not eligible to participate.

Entry Period: The Sweepstakes begins on Tuesday, September 5th, 2023 and ends on Tuesday, September 26th, 2023. All entries must be received by Tuesday, September 26th, 2023 at 11:59 p.m. EST (the “Entry Period”).

How to Enter: To enter, participants must complete the online survey located at http://bit.ly/trhc-emp-sat-surv during the Entry Period. Participants must provide accurate and complete information in the survey, including their name, phone number, and email address.

Prize: One (1) grand prize winner will be selected at random from all eligible entries received during the Entry Period. Prize winner will receive a Getaway Prize Package with an approximate retail value (ARV) of $2,000.00 United States Dollars (USD). The prize package includes:

• Destination: The prize winner can choose from a variety of pre-determined destinations, both national and international, but are subject to availability and travel restrictions.
• Accommodations: A multiple day stay at select locations as chosen by the prize winner.
• Transportation: Round-trip airfare or rental car to and from selected destination.
• Activities: Select activities based upon selected location, e.g., tours, spa treatments, dining experiences, etc.
• Additional Stipend: Other fees and inclusions, e.g., travel insurance, transfers, etc.

The prize is provided by Transitions Healthcare, LLC and Absolute Travel and is non-transferable and non-exchangeable for cash or any other substitute. The grand prize offered in the Absolute Travel Getaway Sweepstakes (“Sweepstakes”) is subject to the following terms and conditions:

Prize Conditions: The prize winner must be available to travel on the specified dates, which must also be approved by Transitions Healthcare, LLC Management.

Prize Acceptance: The selected grand prize winner must accept the prize as awarded and may not request or negotiate cash or any alternative form of compensation in lieu of the specified grand prize.

Non-Exchangeable Provision: The grand prize is expressly non-exchangeable for cash. The winner acknowledges that the prize cannot be redeemed, sold, or exchanged for cash value under any circumstances.

Binding Agreement: By participating in the Sweepstakes and accepting the grand prize, the winner agrees to abide by this non-exchangeable provision and acknowledges that the prize is awarded solely as described, without the possibility of receiving cash or any form of monetary compensation.

Prize Substitution: In the event that the specified grand prize becomes unavailable for reasons beyond the control of Transitions Healthcare, LLC, the company reserves the right to substitute the prize with a prize of equal or greater value, at its sole discretion.

Winner Selection and Notification: The grand prize winner will be selected in a random drawing conducted within 5 business days after the end of the Entry Period. The potential winner will be notified via email within 5 business days of the drawing. If the potential winner does not respond within 5 business days of the initial notification, an alternate winner will be selected.

Odds of Winning: The odds of winning the grand prize depend on the total number of eligible entries received during the Entry Period.

General Conditions: By participating in the Sweepstakes, participants agree to be bound by these Terms and Conditions and the decisions of Transitions Healthcare, LLC, which are final and binding. Transitions Healthcare, LLC reserves the right to disqualify any participant who violates these Terms and Conditions or engages in fraudulent or unlawful behavior.

Publicity: By accepting the prize, the winner agrees to allow Transitions Healthcare, LLC the right to use their name, likeness, and survey responses for promotional purposes without further compensation, except where prohibited by law.

Privacy: Participant information collected during the Sweepstakes will be used for the purpose of administering the Sweepstakes and contacting the winner. Participant information will be subject to Transitions Healthcare, LLC’s Privacy Policy.

Release: By entering, participants release and hold harmless Transitions Healthcare, LLC and its affiliates, officers, directors, employees, and agents from any and all liability for any injuries, losses, or damages of any kind arising from or in connection with the Sweepstakes or the acceptance, possession, or use of the prize.

Governing Law: The Sweepstakes is subject to the laws of the United States of America and any disputes arising out of or related to these Terms and Conditions shall be subject to the exclusive jurisdiction of the courts located in Westminster, MD.

Modification: Transitions Healthcare, LLC reserves the right to modify or terminate this non-exchangeable provision at any time without prior notice.
