A Full Circle of Care: Mary

At Transitions Healthcare Autumn Grove, we pride ourselves not only on the exceptional care we provide to our residents, but also on the daily education and emotional support we extend to their families and loved ones. In December 2024, we had the pleasure of meeting Mary, who visited her husband daily while he was a resident at our facility. During this time, our therapy team built a close rapport with Mary – offering education about her husband’s care, providing emotional support, and creating a foundation of trust through regular, compassionate conversation.

Sadly, Mary’s husband passed away in January 2025. Just one month later, to our surprise, Mary returned to our facility-not as a visitor, but as a resident herself. After slipping on ice outside her home, she sustained a fracture and underwent a surgical repair on her left ankle. Her recovery was further complicated by a recent cardioversion and weight bearing restrictions on the affected leg.

Despite her grief and physical challenges, Mary chose Transitions Healthcare Autumn Grove for her own rehabilitation-an incredible testament to the trust and comfort she felt during her husband’s stay.

Upon admission, Mary required assistance from two staff members for transfers as well as for dressing and toileting. Through comprehensive physical and occupational therapy, she made remarkable progress. In just two and a half weeks, Mary was able to return home-now able to safely manage her daily activities with supervision for transfers and improved independence in dressing and toileting.

Our therapy team worked closely with her family to find safe, step-free access points into her home and ensure a smooth transition. Mary’s journey represents not just physical healing, but emotional resilience-and the power of compassionate, full-circle care. We are honored that Mary entrusted us with her care and proud to have been a part of her continued healing.
