Meet Jessie, who in June 2023 was admitted to Transitions Healthcare Autumn Grove after an extended hospital stay. She had previously completed successful rehabilitation at our facility the previous October following a right hip fracture. Despite her confidence, Jessie faced new challenges after undergoing a bilateral mastectomy when she sadly suffered a fall the next day, fracturing her right hip again and requiring more surgery. Upon arrival to our community, Jessie was unable to use her upper extremities with a walker, necessitating mechanical lift assistance for transfers. Additionally, she had limited mobility and required significant assistance for daily activities. Compounding these challenges, Jessie is deaf and relies on a communication board for interaction.

Through a comprehensive and collaborative rehabilitation effort, Jessie overcame multiple obstacles. With progress, she regained partial upper extremity function and eventually achieved enough mobility to return home after just eight weeks. At discharge, she could perform transfers and ambulation with minimal assistance and manage her own daily activities with support. Jessie’s determination and the support of her family proved that with dedication, anything is possible.
