A Collab For Rehab: Ronald

Transitions Healthcare Autumn Grove is proud to collaborate with Veterans Affairs (VA), offering rehabilitation services for our veterans to support their transition back to community living. In January 2024, Ronald began physical and occupational therapy sessions. Through ongoing communication with the VA, Transitions Healthcare secured authorization for a five-week stay. This allowed Ronald, who initially faced frequent falls at home and struggled with self-care, to return home with the assistance of home health services.
During his stay, Ronald made significant progress. He improved from needing significant assistance for transfers and being non-ambulatory to independently transferring and walking 300 feet with a rollator on various surfaces. He also gained increased independence in performing daily activities. Additionally, he received weekly wound care for pressure areas on his left hip, which healed prior to discharge.
Transitions Healthcare Autumn Grove is so appreciative of the collaboration with Veterans Affairs that improved Ronald’s quality of life and facilitated his successful return to independence within his community.
