Celebration Worthy: Sally

Meet Sally, who was welcomed back to Transitions Healthcare Autumn Grove in August of this year, after being discharged from the hospital following a fall down a flight of stairs at home. Sally had stayed with us earlier in May of 2023, and her family was confident in choosing us again, trusting that our rehabilitation team would support her journey back to independence.
Upon her arrival, Sally required maximum assistance from two staff members for transfers and was non-ambulatory due to a buckling of her lower extremities. Complicating her recovery was a non-weight bearing status on her left second digit, making daily tasks challenging. She was also at a high risk for falls.
Through the dedication and expertise of our Physical Therapy, Occupational Therapy, and Speech Therapy teams, who worked with Sally five times a week, her progress was remarkable. Week by week, Sally grew stronger. By the time of her discharge (and with stand-by assistance), she was able to transfer with a wheeled walker, ambulate up to 220 feet, and perform vital upper body activities of daily living.
Sally’s husband participated in caregiver training, learning how to assist with transfers, gait, and obstacle navigation. The best part? Sally was able to return home the day after her 85th birthday, celebrating this milestone surrounded by her family!
This successful outcome was not just a wonderful gift for Sally, but also for our entire team at Transitions Healthcare Autumn Grove. We’re proud to have played a part in her recovery and wish Sally all the best as she continues her journey at home!
