Meet Robert, who was admitted to Transitions Healthcare Autumn Grove in late July 2025, following a
ground-level fall that resulted in a right intertrochanteric femur fracture. He underwent an ORIF (open reduction internal fixation) procedure to repair the fracture in mid-July. His recovery was further complicated by coronary artery disease, an aortic valve replacement in 2024, and Guillain-Barré syndrome.

Before his fall, Robert maintained an active lifestyle following his valve procedure, reporting no chest pain, or functional limitations. His personal rehabilitation goals upon admission were to walk independently again, return home, and continue driving. At his initial Physical and Occupational Therapy assessment, Robert required assistance with bed mobility and transfers, had decreased range of motion in his right knee, and could only ambulate 10 feet with a front-wheeled walker. He was unable to climb stairs and required extensive help with daily living activities due to weakness.

After just 13 days of intensive physical and occupational therapy, Robert made remarkable progress. He achieved a supervision level of mobility with a front-wheeled walker, was able to climb four stairs with minimal assistance, and could complete his self-care and daily activities with supervision to independence.

Robert was able to safely return home with support from his wife and home care services. Reflecting on his stay at Transitions Healthcare Autumn Grove, he shared: “Therapy was great and everybody was just so nice. It’s amazing-I am a different person from when I came in.”
