Home For The Holidays: Mark

Meet Mark, a 62-year-old man, who was admitted to Transitions Healthcare Autumn Grove following a cervical discectomy and fusion spanning several levels. Although the hospital recommended he return home with assistance from his wife, Mark recognized the value of undergoing rehabilitation to ensure a safe and successful recovery. Choosing our facility for its proximity to home, he was determined to regain his independence before returning to his daily life.
Upon admission, Mark required maximum assistance for bed mobility with the aid of a bed rail, moderate assistance for transfers and all activities of daily living and minimal assistance to walk short distances of 39 feet using a wheeled walker. Complicating his mobility further was the need to wear a cervical collar at all times, except during care.
Through daily physical and occupational therapy, Mark made remarkable progress. After just 17 days, he was able to spend the Thanksgiving holiday with his family and return home with confidence. His wife received comprehensive caregiver training on transfers, gait training, stairs, and car transfers, ensuring both she and Mark felt secure about his return home.
By the time of discharge, Mark had achieved independence in performing all transfers using a wheeled walker, ambulation of 225 feet with supervision using a wheeled walker, and completion of all activities of daily living with supervision. Mark was also provided with a personalized home exercise program to strengthen his lower extremities and maintain the gains he made in therapy.
Mark’s success is a testament to the power of a collaborative rehabilitation process, encompassing thorough evaluation, individualized treatment, caregiver support, and clear communication with medical staff. His journey highlights the importance of a holistic approach to recovery, enabling residents to transition home safely and confidently.
