Transitions Healthcare-Oakland Manor

Assisted Living Facility

November 2021

Visitation Policy

Effective 11/15/21, Transitions Healthcare Oakland Manor will go back to our Pre-Covid-19 visitation schedule. Visiting hours will be daily between 10am and 10pm. Door will lock after 10pm for the security of our residents and staff.

Visitors will be asked to call the facility to schedule a time to visit. We continue to apply core infection control principles to prevent the spread of Covid-19. No visitor will be denied unless the visitor is showing signs and symptoms of an infection. We ask that you do not enter the facility with coughs, sneezes, runny nose, fever, and other symptoms of infection. At this time, we asked that only two visitors at a time per resident. However, if there are more than 2, please allow us time to make arrangements to accommodate for a safe visitation (e.g., masks and space to achieve social distancing). All visitors are required to wear a mask in the halls, common areas inside and outside the building when visiting, particularly when social distancing is not possible. When visitors are visiting their loved ones in the rooms, IF both the visitor and the resident have had both Covid vaccines, then they both can take off the mask inside the room only.

Thank you for your continued cooperation.
