Meet Joan, an 88-year-old resident who had been living independently in a retirement home, faced a sudden health challenge earlier this year. In June 2024, she was admitted to the hospital due to severe abdominal pain. After a 13-day stay in the hospital, Joan was discharged to the care of Transitions Healthcare North Huntingdon. During this time, her ability to care for herself and walk independently had significantly declined.
Upon arrival at our facility, Joan was evaluated by our skilled physical and occupational therapy teams. Recognizing her determination to regain her independence, the therapy team developed a personalized rehabilitation plan tailored to her needs. Over the course of 23 days, Joan worked tirelessly with her therapists, focusing on improving her strength, mobility, and daily living skills. Her resilience and commitment to her recovery were evident as she made steady progress, regaining her ability to function independently in most areas of daily life.
Thanks to her hard work and the dedicated support of our therapy team, Joan successfully regained her independence and was able to return to her independent living community in July. Meet Ethel, whose journey at Transitions Healthcare North Huntingdon is nothing short of remarkable. After undergoing a below-the-knee amputation, Ethel faced the daunting task of relearning many aspects of her daily living. Thanks to her unmatched determination combined with the dedicated support from our rehabilitation team, she was able to achieve all her activities of daily living (ADL) goals and transition back home in just 28 days.
During her stay, Ethel mastered the ability to ambulate short distances with a wheeled walker, demonstrating incredible progress and mobility. She diligently worked on her self-care routines, successfully completing them with newfound independence. Her ability to transfer to various surfaces—including the bed, wheelchair, and armchair—was outstanding. Each milestone was met with determination, and each success brought her closer to her goal of returning home.
At the end of last month, Ethel returned home to the loving embrace of her husband and son, equipped with the skills and confidence to continue her rehabilitation journey. Her next step is to complete her prosthetic training from home at an outpatient clinic, where she will learn to use her new prosthetic leg.
