After being hospitalized for an insidious pneumothorax requiring thoracocentesis and a chest tube, Frank faced a challenging journey ahead. Adding to his recovery, he was still following precautions from a recent hip replacement.

    Frank arrived at Transitions Healthcare Shook Home‘s skilled nursing facility needing more than minimal assistance for his activities of daily living (ADLs), transfers, and ambulation. With the support of our dedicated therapy team and his own hard work, Frank steadily regained strength and independence.

    After just a 7-day stay filled with physical and occupational therapy, Frank was successfully discharged home with his wife by his side. He is now fully independent in his daily life and moves about freely using his rolling walker.

    Frank’s determination and positive attitude have been an inspiration to everyone who had the pleasure of supporting him. We wish Frank and his wife all the best as they continue this new chapter at home!

After experiencing a fall at church that resulted in multiple pubis fractures and a scalp contusion, she faced significant hurdles. Her journey to recovery was further complicated by her diabetes, but Lois was ready to tackle each obstacle head-on.

    During her 9-day stay at Transitions Healthcare Shook Home‘s skilled nursing facility, Lois participated in physical and occupational therapy. Initially, she required moderate support and assistance with activities of daily living (ADLs), transfers, and ambulation. Through hard work, dedication, and a positive spirit, Lois quickly progressed in therapy, regaining her strength and independence.

    Upon discharge, Lois returned to her home, living independently. She can now return to her daily life, complete light housekeeping tasks, and move about confidently using her rolling walker for ambulation. With the support of her loving daughter, Lois is thriving and back to enjoying life.

  • Never Too Old To Thrive: Haynard

    At 95 years young, Haynard‘s resilience and determination inspire us all! After a long journey of limited mobility due to a left total knee arthroplasty (TKA) revision and several health challenges, including post-hemorrhagic anemia, Bell’s Palsy, and a hearing deficit, Haynard was ready for a fresh start.

    In January 2024, he arrived at Transitions Healthcare Shook Home‘s skilled nursing facility requiring maximum assistance for transfers, ambulation, and activities of daily living. With weight-bearing as tolerated (WBAT) and a dedicated team to support him, Haynard began his recovery journey.

    Over the course of his 31-day stay, Haynard showed remarkable progress. Through physical therapy, occupational therapy, and a lot of personal determination, he steadily regained strength and independence. By February 2024, Haynard was able to return to his personal care home requiring just one assist for transfers and ambulation-and he has been thriving ever since!

    Haynard’s story illustrates the true power of perseverance and the importance of teamwork in achieving recovery goals. Transitions Healthcare Shook Home is so proud to have been part of his journey, and we wish him continued success and independence!

  • A Remarkable Climb: Melvin

    Meet Melvin, a 75-year-old gentleman, who came to Transitions Healthcare Shook Home following a challenging journey. After a fall in the community, Melvin was hospitalized and diagnosed with a displaced comminuted fracture of his right patella. Following orthopedic surgery, he faced a long road to recovery, including being non-weight-bearing for 6-12 weeks.

    Melvin’s situation was further complicated by the residual effects from a previous stroke, which left him with left-sided hemiplegia, and the need to use a knee immobilizer. Upon arrival, Melvin required significant assistance, including the use of a mechanical Hoyer lift for transfers.

    However, through perseverance and the collaborative efforts of our interdisciplinary care team — including therapy, nursing, and physicians — Melvin made remarkable progress! Over 44 days, he overcame these barriers and steadily improved his functional abilities.

    By the time of his discharge, Melvin had achieved independence with a rolling walker for transfers, ambulation, and activities of daily living. He even mastered independently climbing 13 steps, a significant milestone in his recovery.

    Thanks to Melvin’s determination and the support of his sister-in-law, he was able to return home safely and independently. His story illustrates the true power of teamwork, tailored care, and the resilience of the human spirit.

  • A Journey Home: Dennis

    Meet Dennis, a 68-year-old male, who came to Transitions Healthcare Shook Home after a 29-day stay in an acute care hospital, during which he required ECMO support due to pneumonia (PNA) and acute renal failure. Despite facing significant challenges, including initial reliance on a mechanical Hoyer lift, COPD, being non-ambulatory, and dealing with a urostomy complicated by severe pain from ECMO complications, Dennis made remarkable progress with the collaborative efforts of our interdisciplinary care team comprised of therapists, nurses, and physicians.

    Through dedicated therapy and support, Dennis achieved significant milestones, enabling a safe discharge home with his wife. He transitioned from dependent use of a mechanical hoyer lift to independent mobility with a rollator for transfers, ambulation, and activities of daily living (ADLs). This successful outcome is a testament to Dennis’s determination and the comprehensive care provided by our team.

    We are thrilled that Dennis has regained his independence and returned home to continue his recovery journey alongside his loved ones. Congratulations, Dennis!
